   Data includes: physical and neurological exam findings, neurocognitive test scores, survey assessments, personal and family history, positive/negative family history of dementia, personal demographics, selected genotypes, age at service evaluations, age at onset, age at death, clinical diagnosis, neuropathology diagnosis and inventory information (when available), health status, medications, laboratory tests, MRI images and volumetrics, contact information required to manage ongoing clinical care and study operations.
